VNS Health is seeking compassionate and dedicated Home Health Aides to join our Personal Care & Service team, supporting patients and families throughout New York City. As a Home Health Aide with VNS Health, you will provide one-on-one care in the comfort of patients’ homes, helping individuals maintain their dignity, independence, and quality of life. You will assist with activities of daily living such as bathing, grooming, dressing, toileting, and mobility, while also offering companionship, emotional support, and respectful, person-centered care. Working under the supervision of a registered nurse or other licensed clinician, you will follow individualized care plans, provide non-clinical support such as medication reminders, observe and report changes in a patient’s condition, and help ensure a safe, clean, and comfortable home environment through light housekeeping, meal preparation, and support with errands as allowed by policy. In this role, you will collaborate closely with an interdisciplinary care team, including nurses, therapists, social workers, and care coordinators, communicating clearly about patient needs and progress. You will practice strong infection control and safety procedures, use proper body mechanics and transfer techniques to reduce the risk of injury, and respond appropriately to urgent or unexpected situations by following VNS Health protocols. Because you are working in the field, you will travel between patient homes within assigned neighborhoods or boroughs, adapt to a variety of home settings and family dynamics, and demonstrate cultural sensitivity with each patient you serve.
