VNS Health is seeking compassionate and reliable Home Health Aides to provide high-quality personal care and daily living support to patients in their homes across New York City. As an essential member of our Personal Care & Service team, you will help individuals maintain their dignity, safety, and independence while remaining in the comfort of their own homes. You will work under the direction of licensed nursing and clinical staff to deliver patient-centered care consistent with VNS Health’s long-standing commitment to excellence and innovation in home and community-based healthcare. In this role, you will assist patients with activities of daily living, including bathing, dressing, grooming, toileting, and mobility support, while ensuring comfort and respect for each person’s preferences and cultural background. You will provide safe assistance with transfers, positioning, and walking, using proper body mechanics and any prescribed assistive devices. You may help with simple meal preparation, feeding, and maintaining a clean, safe environment by performing light housekeeping tasks related to the patient’s care. Home Health Aides at VNS Health observe and report changes in a patient’s condition, such as appetite, mobility, skin integrity, or behavior, to supervising nurses or care managers to support timely clinical decisions. You will offer companionship, emotional support, and encouragement, helping patients and their families manage daily routines and feel confident in the care they receive. Following individualized care plans, you will provide medication reminders as directed, adhere to infection control practices, and respect all safety standards and confidentiality requirements. You will represent VNS Health in the community, traveling to patients’ homes throughout New York City and adapting to a variety of home environments with professionalism, patience, and cultural sensitivity. Collaboration is central to this position: you will communicate regularly with nurses, therapists, social workers, and other members of the care team to ensure coordinated, holistic support for each patient. You will document visits and services provided accurately and in a timely manner, using paper or electronic tools according to agency standards.
