Peoples Home Health, LLC is seeking a compassionate, part-time Hospice Chaplain to join our Healthcare & Medical Services team. In this role, you will provide spiritual, emotional, and bereavement support to patients and families receiving hospice care in their homes and in community settings. As a Hospice Chaplain, you will visit patients and families in their homes and in designated care locations to assess spiritual and emotional needs, offer counseling, prayer, and supportive presence, and help patients and families find meaning, comfort, and peace at end of life. You will collaborate closely with the interdisciplinary hospice team—including nurses, social workers, aides, and physicians—to integrate spiritual care into each patient’s individualized plan of care. You will conduct spiritual assessments, document interventions and care plans in the medical record, and provide ongoing follow-up as needs change. The Hospice Chaplain will facilitate or coordinate rituals and spiritual practices according to each patient’s faith tradition or personal beliefs, and will support those who identify as spiritual but not religious, or who do not hold specific faith beliefs. You may help coordinate visits from community clergy and faith leaders and serve as a liaison between the hospice team and local faith communities when appropriate. The chaplain will offer anticipatory grief support to patients and families, assist families in preparing for loss, and provide bereavement support after death in collaboration with the hospice bereavement team. You may lead or participate in memorial services, remembrance activities, and educational sessions about grief and coping. Sensitivity to diverse cultures, beliefs, and backgrounds is essential. Aligned with the culture of Peoples, you will foster a supportive, safety-focused approach in all patient encounters, follow all hospice and healthcare regulatory requirements, and maintain strict confidentiality. You will participate in interdisciplinary team meetings, case conferences, and quality improvement activities, and you will communicate clearly and respectfully with patients, families, colleagues, and community partners.
